LAMBDA Aloha/PFLAG Kaua’i Scholarship Application Checklist

Thank you for your interest in the LAMBDA Aloha/PFLAG Kaua’i Scholarship. The criteria for the scholarship is as follows:

·      Applicants must be a resident of Kaua’i

·      Applicants must be a first time college student

·      Applicants must be an active member of Lambda Aloha, PFLAG Kaua’i, OR Gay/Straight Alliance (GSA).

*If not a member of Lambda Aloha, PFLAG Kaua’I, or a GSA, you must be an active member of an agency, group, or organization that encourages and participates in education, advocacy, and equality related to Lesbian, Gay, Bisexual, and Transgendered (LGBT) issues. Please describe this agency, group, or organization as well as your affiliation/role in detail in section D.

We wish you the very best in your educational endeavors. In addition to the application, we have included a checklist for your convenience to ensure that all applicable materials are received. Please ensure that all items are received no later than, MAY 15, 2013. Applications received after the deadline will not be considered. Scholarship will be awarded June 9^th, Kaua’i PRIDE weekend.  Please send all completed applications to:

 Completed application form that is dated AND signed

  ONE Letter of Recommendation from someone who you feel knows you well. (This could be a teacher, coach, mentor, etc. Please refrain from using immediate family members). The letter should be in a sealed envelope with the author’s signature across the seal.

 Please ensure that any additional sheets are securely attached to the application and have your FULL NAME and page number as a header (i.e. Last Name, First Name page 1 of 6).
